PSYCHIATRY PROVIDERS:

Ramteen Rezai, MD

Dr Rezai is a board-certified general psychiatrist. He completed his residency training at New York Medical College at Westchester Medical Center, as well, as a two-year certification course in psychodynamic psychotherapy at New York Medical College Psychoanalytic Institute. He provides comprehensive, high-quality outpatient mental health services to a broad range of individuals and their families.

 

At TAK Center for Mental Health, Dr. Rezai treats patients with a variety of psychiatric disorders including, but not limited to, anxiety disorders, mood disorders, psychotic disorders, ADHD and sleep disorders. He offers a compassionate, collaborative and multi-faceted treatment approach to patient healing, leading to a better and long-lasting quality of life.

Jeremy Cohen, PMHNP 

Jeremy Cohen is a Psychiatric Mental Health Nurse Practitioner, Board Certified and graduate with a Master's Degree from the University of Colorado, Denver. His previous experience as an RN in Intensive Care Unit settings helps promote facets of care that not only pertain to psychiatric related treatments but lend insight into medical diagnoses and their influences with regard to potential treatment approaches. Having a BA in Psychology from the University of Miami enhances therapeutic rapport with use of Dialectical Behavioral Therapy and Solution Focused Brief Therapy as it relates to goals of care and building a trusting relationship. Jeremy holds a firm belief that every client's life journey is unique and deserves a personalized approach with incorporation of holistic practices, identifying social support systems, and grounding in personal values that is best understood through the lens of an individual's personalized narrative. His evidenced-based approach inspires a comprehensive treatment plan that embraces the role of psychotherapy, lifestyle modifications, and medication management. 

Amanda Liberty, PMNHP

Amanda (Ortiz) Liberty, MSN, RN, PMHNP-BC, is a certified psychiatric mental-health nurse practitioner and a first-generation college graduate. She holds degrees from Middlesex Community College, University of Massachusetts (Lowell), and Regis College with outstanding achievements.

 

With over a decade of experience in mental health, Amanda has made a significant impact in supporting veterans as the Director of Community Outreach for Veterans Advocacy Services. She has also organized successful fundraising events for the Semper Fi Fund and worked as a Registered Nurse at an inpatient psychiatric treatment center in Devens, MA.

 

Amanda's dedication to community service has earned her multiple citations from the Commonwealth of Massachusetts House of Representatives for her philanthropic efforts. She received specialized training as a PMHNP at VA Boston Healthcare System, alongside and in collaboration with Harvard South Shore Residents on areas such as PTSD, psychopharmacology, and trauma-informed care.

 

Her extensive background includes providing both inpatient and outpatient psychiatric and substance use treatment. She has worked at a community hospital in Lowell as well as an outpatient clinic, delivering community-based care. Currently, she serves as an addiction-psychiatry consultant at Massachusetts General Hospital, a role that brings her great fulfillment.

 

Amanda's passion lies in helping individuals struggling with trauma, anxiety, depression, mood disorders, and substance use disorders. She prioritizes patient-centered care and aims to remove stigmas associated with psychiatric and substance use treatment by approaching her patients with compassion and nonjudgment.

 

In addition to her professional achievements, Amanda is a certified RYT-200 and incorporates holistic wellness into her treatment plans. She strongly believes in the healing power of nature and enjoys exploring the beautiful landscapes of New England with her family and two dogs.

Karen Sullivan, LICSW

Karen Sullivan has been a licensed independent clinical social worker for over 25 years and has worked in a number of different settings, which allowed her to gain knowledge and experience to address any presenting concerns. Karen graduated from Boston University’s prestigious program and in addition, works at her own private practice. Karen supervises the psychotherapy clinicians to ensure high quality treatment.

Deborah Parsons, Licensed Mental Health Counselor

Debbie is a Mental Health Counselor who is licensed with the Commonwealth of Massachusetts and the state of New Hampshire. Debbie has worked with individuals, couples and families of all ages and with many mental health challenges, including Depression, Anxiety, Bipolar Disorders, OCD, Trauma, Mood Disorders and others as well as various life challenges including relationship issues, stress, women’s issues, gender identity issues, coping skills and more. She now meets with adolescents and adults for individual and couples therapy.

 

Debbie offers a welcoming, gentle and laid-back approach that is holistic, creative and collaborative. Her goal is to work on understanding you and your challenges and finding practical solutions that work specifically for you using a variety of therapeutic methods, including CBT, DBT, Trauma informed therapy, mindfulness, EFT, bibliotherapy, talk therapy and others. Debbie earned her degree from Rivier University, Nashua, NH. She completed her internship at Keene State College and has been working in the field since 2013. Debbie is an animal lover, who enjoys time in nature, being creative and spending time with family and friends.

Tamara Perini, LICSW

Tamara Perini is a licensed independent clinical social worker and provides psychotherapy for individuals, couples, and families ranging from adolescents to older adults. Tamara focuses on empowerment and self-change to help motivate a more positive, stable, and meaningful life. Tamara works alongside Karen Sullivan to supervise the social worker clinicians at TAK.

Michelle Chouinard, LCSW

Michelle Chouinard, MSW, LCSW is a Licensed Clinical Social Worker with a Master’s degree in Social Work from Boston University, with an emphasis in trauma-informed care and addiction. Michelle has experience providing clinical services to adolescents, adults, families and groups. Michelle has worked in a variety of therapeutic settings to include the military, criminal justice system, alternative school, residential, and intensive out-patient. Michelle’s passion is to walk alongside others to foster hope while assisting them in healing from trauma, building self-worth, improving communication and reaching their therapeutic goals.

Megan Popham, LMHC

Megan is a Mental Health Counselor who is licensed with the Commonwealth of Massachusetts and received her graduate degree in clinical mental health counseling from Southern New Hampshire University. Megan also has an associate degree in criminal justice along with a bachelors degree in forensic psychology.  Megan has worked with individuals of different ages that deal with the mental health challenges of Bipolar Disorders, Anxiety, Trauma, Depression, Schizophrenia, Substance Abuse, and Mood Disorders. Megan also works with individuals in public service such as police, fire, emergency medical services, and correctional officers and the challenges that come with those professions.   Megan has worked in community mental health and inpatient hospitalization settings.


Megan offers a laid-back approach to therapy by utilizing person-centered therapy where the goal is for the individual to identify trauma, develop and utilize effective coping skills, minimize negative feelings, increase self- acceptance and self-esteem, and to gain a better understanding of oneself.

Malanne Slater Coleman, LCSW

Malanne is a dedicated and compassionate Licensed Clinical Social Worker (LCSW) with a deep passion for helping individuals navigate life's challenges. With over 17 years of experience in the field of social Work, she has the privilege of working with diverse clients, each with their unique struggles and strengths.

 

Malanne's therapeutic approach is rooted in empathy, active listening, collaboration, and a non-judgmental environment where you can openly explore your feelings, thoughts, and concerns. She believes that every person has the capacity for growth and healing, and she's here to provide the support and guidance to help you achieve your goals.

Katherine Brodmerkle, LMHC

Katherine is a Mental Health Counselor who is licensed with the Commonwealth of Massachusetts. She received her Master’s Degree in Mental Health Counseling and Expressive Arts Therapy from Lesley University in 2017. Katherine has worked with individuals of different ages living with the mental health challenges, with a special focus on working with adolescents and transition age young adults in community, inpatient, and higher education settings. Katherine approaches therapy through a relationally based, person-centered lens. She practices a combination of somatic and cognitive approaches of trauma informed care, with special interests in working with members of the LGBTQIA+ and neuro-diverse populations.

Katelyn DePatsy, LCSW

Katelyn is a Licensed Clinical Social Worker with a Master's degree in Social Work from Anna Maria College. Katelyn has experience working with individuals of all ages seeking support with depression, anxiety, trauma, substance use, relationship issues, and life transitions. Katelyn strives to create a safe environment without judgement and offers an empathetic, collaborative approach.

Dr. Frank Vargo, PhD

Dr. Frank E. Vargo is a licensed psychologist and registered health service provider, a clinical and developmental neuropsychologist, and a licensed school and educational psychologist. He is an internationally recognized expert in the inter-disciplinary fields of applied clinical and educational psychology, neurodevelopmental disorders and related clinical/medical syndromes, learning disabilities and special education, and clinical child psychology and development.     Dr. Vargo is a highly respected adult, child, and family therapist, as well as a licensed educator, counselor, and clinical and educational administrator and supervisor.
